Application Checklist

We are pleased to have you apply for a job with Stidham Trucking, Inc. Some requirements for this job are given below. Please read each requirement carefully. Place your initials on the blank to the left of each requirement that you meet.

	Must be at least 23 years of age.
	Must possess a valid CDL Class A license with appropriate endorsements.
	Minimum two years experience driving commercial vehicles (heavy truck and trailer combinations) over the road.
	Minimum one year flatbed experience with a basic knowledge of how to secure and tie down loads.
	Must be able to read and write English fluently.
	Must be able to pass a DOT physical examination.
	Must meet Federal Motor Carrier Safety Regulations, drug screening requirements and must agree to submit to controlled substance and alcohol testing as required by the Federal Motor Carrier Regulations.
	Must be familiar with Safety Regulations prescribed by DOT.
	Must be able to properly maintain drivers daily logs.
	Must be able to drive 70 hours in an 8 day period.
	Must have general knowledge of all State Traffic laws, Load Restrictions, and other regulations pertaining to interstate/intrastate movement of goods.
	Must maintain a clean appearance, demonstrate a professional attitude.
	Must obey all company safety rules and regulations.
	No major traffic violations in the past three years: Reckless/Careless driving. Driving while intoxicated or under the influence of drugs. Excessive speeding as defined by DOT, any single offense for any speed 15 mph or more above posted speed limit. Hit and Run citation
	None of the above with any subsequent violation in the last 4 years.
	No more than 4 moving violatins within the last 3 years and no more than 2 moving violations within the last 12 months.
	No current license suspension/revocations.
	No preventable accidents involving a fatality, bodily injuries treated away from the accident scene, or disabling damage to a motor vehicle. (Disabling damage means damage which precludes departure of any motor vehicle from the scene of the accident in its usual manner in daylight after simple repairs. This includes damaged vehicls which are drive-able, but would incur further damag if so driven.)
I understand that the information in this application will be used and that prior employers will be contacted for the puropses of investigation as required by 391.23 of the Federal Motor Carrier Safety Regulations. I certify that the above information is correct and given of my own free will and accord. I also certify that I have not knowingly withheld any information which will reflect upon my fitness as a candidate for the position of which I am applying. I understand that giving false information or withholding pertinent information can disqualify me for this position. I also understand that if I "lease on" and I am found to have given false information this will serve as cause for my removal from the job anytime it might be revealed that false information was given. In consideration of my leasing on, I agree to conform to the rules of this Company and the Federal Motor Carrier Safety Regulations and any other relevant Regulations and Laws. I understand and acknowledge that nothing contained in any contract and/or agreement received by me at the commencement of my "leasing on" nullifies or modifies the foregoing.
